Diabetes/Insulin Information regarding DOT/CDL physicals:
No longer need exemption for Diabetes. Now the Form MCSA-5870 is required to be filled out prior to receiving your DOT/CDL exam. A copy of your HgA1C is needed. Must be 7.0*. Three months of blood glucose self monitoring records must be provided.. If you do not have 3 months of documentation, then your DOT/CDL physical can only be given for three months and you must recertify and bring in your required documentation of three months of blood glucose monitoring.
Certification is prohibited:
Stage 3 or 4 Diabetic Retinopathy
If your CDL was received in Canada or Mexico
Diabetic complications such as Kidney failure or insufficiency
If you have an episode of hypoglycemia with a loss of consciousness or receive help from another individual for severe hypoglycemia, you will require a new exam.
