Diabetes/Insulin Information regarding DOT/CDL physicals: 

There is no longer a need for an exemption for Diabetics on insulin. Now the Form MCSA-5870 is required to be filled out prior to receiving your DOT/CDL exam. A copy of your HgA1C is needed. Must be 7.0* or better. Three months of blood glucose self monitoring records must be provided if you do not have a recent A1C. Not having these items can cause your physical to be delayed or denied. If you are on oral medication, the A1C guidelines are the same. 

Certification might be prohibited for: 

Stage 3 or 4 Diabetic Retinopathy

If your CDL was received in Canada or Mexico

Diabetic complications such as Kidney failure or insufficiency

 

***If you have an episode of hypoglycemia with a loss of consciousness or receive help from another individual for severe hypoglycemia, you will require a new exam.
